U.S. Citizenship and Immigration Services (USCIS) requires the I-693 form for all those applying for a permanent residency (a green card).

Pricing:
Our special USCIS procedure can save you as much as $300 to $400 dollars by working with the NYC Department of Health to receive vaccines and a chest x-ray IF you qualify.
Our office USCIS fee for adults is $165.00 …… and $130.00 for children under 12 years of age.
Our special USCIS price includes your USCIS physical exam, your USCIS health history, your required PPD skin test for tuberculosis and the careful completion of all your USCIS-693 paperwork.
Please be aware that there is an additional $20.00 fee for taking a blood sample and processing IF taking an in-office blood sample is deemed necessary. You may also receive a SEPARATE bill from the blood lab testing company (SHEIL LABS) for about $25.00, a week or so after your examination.
Forms of Payment: We accept cash, MC/Visa cards, American Express card or the Discover card or a personal check. Payment in full is expected at the time of your initial visit. We do not accept insurance for the USCIS medical exam. Your health insurance plan MIGHT cover the cost of any outside lab testing fee.
What to Bring:
Please bring ANY vaccine records you might have and your passport. You can also FAX any vaccine records to our office in advance of your visit to FAX # (646) 349 – 5328.
Language and Translation:
Please bring a friend or relative with you who speaks your language as well as English so they can translate for you.
Processing Time: The USCIS 693 exam and paperwork usually takes three to five business days to complete depending on your test results.
