Pricing:
The price $125.00 for adults and $100.00 for Children under 12 years of age. This includes a USCIS physical exam, a USCIS health history, a PPD skin test and the completion of your USCIS-693 paperwork handed to you in a sealed envelope. Be advised that there is an additional $20.00 fee for veni-puncture (blood drawing) and blood sample processing IF an in-office blood test is deemed necessary.
Lab fees: Any laboratory fee for any blood tests done in the office is the sole responsibility of the patient. If you have a blood test in the office of Dr. Ores, YOU will receive a bill directly from a lab testing company (eg.; Shiel Labs). Dr. Ores will make every effort to keep the cost of lab testing to an absolute minimum.
For example, a simple blood test for Varicella (Chicken Pox) immunity costs about $15.00 dollars, but this blood test will save you the expense of a $125.00 Varicella vaccine.
Also, if you happen to have medical insurance, it is very likely your insurance will cover lab blood testing done in the office of Dr. Ores.
IMPORTANT NOTE:
The $125.00 initial fee this DOES NOT include cost of any vaccinations (if vaccines are needed) or of any blood testing (see below).
Forms of payment: We are happy to accept cash, MC/Visa cards, American Express card or the Discover card or personal check. Payment in full is expected at the time of your initial visit.
Insurance & Payment Policy: Payment in full is expected at the time of your initial visit. However, you will receive an official medical receipt, which you can submit to your insurance company for possible reimbursement. Please be advised that private insurance companies in the USA usually DO NOT pay for the 693 immigration medical exams.
Vaccines:
Please bring ANY vaccine records you have with you when you visit. Vaccines are very expensive. Adults need three vaccines. Children usually need more than three vaccines. Dr. Ores will work with you so you MAY be able to receive your required vaccines at NO CHARGE (if you qualify) through the NYC Department of Health. You can also receive a Chest x-ray (if you have a positive PPD skin test) at NO CHARGE through the NYC DOH if you qualify. This approach may save you hundreds of dollars in expense. The NYC Department of Health offers the MMR vaccine and the DT vaccine at no charge. If you have proof of no immunity to chicken pox (this requires a blood test at the office of Dr. Ores) the DOH will also give you the required Varivax (chicken pox) vaccine at no charge.
IMPORTANT NOTE:
Do not go to the NYC department of health for vaccines until AFTER your initial visit and evaluation with Dr. Ores. We have to discover which vaccines and tests you may need.
What to Bring with you to Dr. Ores:
1. Your original passport. Not a photocopy.
2. Any vaccination records you may have. You can also fax any vaccine records from any country to David J. Ores, MD at FAX # 646-349-5328 on a 24-hour basis.
Language & Translation: Please bring a friend or relative with you who speak your language as well as English so they can translate for you.
Processing Time: The USCIS 693 exam and paperwork usually takes three to five business days to complete depending on your test results.
